As there can be false positives,I always check everything in my Akismet spam filter on my free-hosted blog before pressing "delete".
I have also noted that since March the number of false positives occurring have decreased substantially. The work done to improve Akismet has been successful as is witnessed by the lack of posts to the wp.com support forum on this issue.

On my self hosted wordpress install on my domain I use Defensio and I always check that filter as well before pressing "delete".

I detest captcha's and rarely comment on blogs with them. I have had very few problems with akismet if one or two commenters who routinely end up there, once I release them a few time akismet seems to remembers they are not spam.

reCaptchya ended up being very cumbersome to my readers and I still had spam in y akismet so ... back to only akismet.
