iChat AV - What a pain

iChat is great, the features are even better, especially in Leopard... if only...
IF ONLY WE COULD GET THE DAMN THING WORKING!
I don't get it, MacOS is all about being user friendly yet there is nothing user friendly about their network configuration for iChat.
I configure advanced router systems for a living, yet I can't get iChat A/V to work. I'm used to working with Linux based routers, Cisco Routers, SOHO routers, ...
I tried every configuration, even exposed the iMacs on both ends completely to the internet, disabled every bit of firewall in between en tested it with bunch of tools. Every test indicated both machines were directly accessible through the internet... but still iChat A/V wouldn't work.
I don't know what they expect us to do... buy
a separate ADSL connection to put a single iMac
on it and hope we'll be able to use iChat AV ?
I'm sorry for the ranting... I'm in a bad mood

I've tried this on multiple locations with multiple routers and configurations. I manage networks and design firewalls for a living...
And I just can't figure it out. Every possible test indicates both ends are exposed 100% to the internet. Every service on both macs is reachable from the internet.... but I really can't get iChat AV to work. -
In general Mac to Mac seems to work pretty well, but cross platform and iChat to AOL video in particular is abysmal and so iffy and flakey. Routers and Firewalls can vary from person to person and connection to connection. Sometimes I think the best solution to iChat problems is just to use SKYPE!
